I understand and agree that this request is subject to appropriate verification and that this request may be refused if such verification fails in accordance with the California Consumer Privacy Act (CCPA).
I understand and agree Mederma.com’s Privacy Policy and Terms of Service govern my request to the extent they are not in conflict with the CCPA.
I understand that Mederma will endeavor to acknowledge receipt of my request within 10 days of my request, and comply within 45 days of my request. I further understand that if circumstances warrant, it may take up to 90 days for Mederma to comply with my request.
